The Garry Hurvitz Centre for Community Mental Health at SickKids (GH-CCMH) is a child and youth mental health centre integrated with The Hospital for Sick Children (SickKids). GH-CCMH is also a core service provider within the Toronto region of Child and Youth Community Mental Health Providers. As such, the Centre has an ambitious agenda to provide the highest quality evidence-informed care offering a range of services from prevention and early intervention through counselling and therapy, assessment and intensive supports. Services are provided in both in-person and virtual formats. We are currently seeking applicants for a full-time 1.0 FTE Case Manager.
This position is dedicated to the transition of children, youth, and their families from hospital to community based mental health services and supports including the Hospital for Sick Children, CAMH, and GH-CCMH. The position will work collaboratively with relevant system partners to support the clinical transition pathways. This position will provide service planning, navigation and brief supportive counselling to children and youth experiencing mental health concerns, and their families, to ensure they move in through and out of publicly funded mental health services in a timely and seamless manner.
KEY RESPONSIBILITIES
- Provide developmentally informed transitional support for children, youth and families referred between SickKids and CCMH
- Provide family education, counselling and support through brief interventions.
- Facilitate multiple care aspects (e.g. case coordination, information sharing, implementation of practical clinical strategies, system navigation, caregiver support group).
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, patient and family centered and meaningful to the adolescent.
- Develop effective working relationships with relevant hospital and community based mental health service system partners to collaborate/communicate throughout the entire case management process.
- Practice in a manner that is consistent with the philosophy of continuous quality improvement and evidence-based practice.
- Prepares client documents and submits reporting information according to established procedures and deadlines.
- Maintains up-to-date knowledge of therapeutic services and best practices within the industry.
Qualifications
- Master of Social Work or equivalent in a mental health discipline and a minimum of three years’ related clinical experience or equivalent combination of education and experience
- Membership in a professional College
- Good verbal and written communication skills to provide therapeutic services to clients and families
- Good problem-solving skills to analyze complex client issues and recommend solutions
- Demonstrated skills in therapeutic engagement
- First Aid/CPR is an asset
- Good knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint)
- Ability to incorporate values of a culturally competent practice of diversity and inclusiveness in all aspects of work
- Ability to work flexible hours, including evenings and weekends

- Complies with the organization’s vision, policies and procedures
- Performs other tasks as assigned by management
Effort
- Concentration and creativity required daily to prepare documents, listen, attend meetings, mediate conflicts, develop transition plans and brief treatment plans, etc.
- Sits at a desk and in sessions with clients in order to perform the duties of the job, as well as move objects or lift books for filing
