Strides Toronto is a multi-service agency providing a wide range of exceptional programs and services that improve the mental, social, and physical health of infants, children, and youth from the pre-natal stage to age 29, and their families.  With a range of community, residential and treatment services our work helps to strengthen social and emotional well-being, for culturally diverse clients to move forward through adversity and develop abilities and skills to reach their full potential.   We provide individual, group and family interventions that include mental health counselling, autism services, education, outreach and referral, early intervention, day treatment and community support. ABOUT THE ROLE

The Social Worker (WIT) is a full-time contract position that is an interim clinical case manager who develops, co-leads and participates in building teams, to design treatment plans for service providers who are supporting youth with complex clinical profiles or unique service situations.  This is a contract position until March 2023.

Salary range:     $64,573 to $74,875 per annum

Other:                 This role is a part of the OPSEU Local 567 bargaining unit.

WHAT YOU WILL DO

– Consult, correspond and collaborate with service provider teams to design, implement, measure, and review individualized clinical treatment plans for youth with complex clinical profiles (multiple or unique diagnoses), or youth with complex clinical situations (the service does not exist, or it does exist but there are barriers to accessing it).
– Monitor and address obstacles and barriers to client’s treatment plan through ongoing communication with team members and implementing supports to overcome obstacles/barriers in a timely manner
– Source and retain client support resources
– Ensure client information is recorded accurately and in accordance with Ministry of Health guidelines, agency, and accreditation standards.
– Identify systemic challenges in program/treatment delivery and advocate for change and/or additional supports for these treatment elements with organizational leaders/managers or ministry contacts.
– Provide solution-focused, brief therapy supports to children, youth and young adults from birth to age 29, and/or families within the What’s Up Walk-In Clinic (twice monthly).
– Train, direct and guide MSW social work student placements in accordance with University and agency guidelines.

THE “MUST HAVES”

The following education, professional memberships and knowledge are a requirement for this role:

– Master’s degree in Social Work, Psychology or equivalent field
– Membership in the appropriate professional regulatory body
– 3 years of related experience
– Extensive knowledge of mental health disorders and diagnoses
– Familiarity with and exposure to the Diagnostic and Statistical manual
– Experience and exposure -working with youth in the developmental services sector.
– Ability ease and comfort in coordinating and leading cross sectoral teams for the purpose of clinical treatment interventions.
– Knowledge of other services and interacting with staff in management and senior levels
– Awareness of the role of diversity, equity, and inclusion in the workplace and within the community being served by the position.
– An understanding of Anti-Black Racism, anti-oppressive/anti-racism perspective in the context of the social determinants of health when working with clients and their families.
– Ability to recognize and identify situations where incidents of and imbalance of power and privilege may occur and can result in possible negative impacts for the client.

THE ‘NICE TO HAVES’

While not required, preference will be given to candidates with the following:

– Knowledge of the cultural diversity of communities served by the agency.
– Fluency in another language(s), including sign language.

BEFORE STARTING WITH US

As our work puts us in direct contact with youth and vulnerable persons in our community, with health and safety as a priority, the successful candidate must complete or obtain the following before commencement of their employment:

– Full vaccination against COVID-19 or have received an exemption from the organization’s policy.
– A Vulnerable Sector police check.
