ABOUT THE ROLE
Reporting to the Manager of Day Treatment, the Transitions Facilitator is responsible for providing assessments, case management, treatment planning, advocacy and support to children and youth in the Day Treatment Program who require complex service with transition planning. In this role you will apply strength-based and solution-focused approaches during transition planning and community integration, including working within a multidisciplinary approach for treatment, intervention and transition planning that is consistent and complimentary to school-based mental health intervention, integrated agency model, and the school community. The Transitions Facilitator will collaborate with various staff and departments to develop and implement individual transition plans, including ensuring all related and mandatory documentation is completed in accordance with licensing requirements and internal policies and procedures.
Employment Status: Permanent Full-Time
Salary Range: $56,014 to $58,323
Location: 11225 Leslie Street, Richmond Hill; this position will require travel within York Region.
Hours of Work: Thirty-seven and a half (37.5) hours per week.
A LOOK AT YOUR DUTIES AND RESPONSIBILITIES:
• Participate in the development and maintenance of a transitional framework guided by best practices, internal policies and procedures, licensing requirements and accreditation standards, when working with children, youth and their families within the Day Treatment Program, and other programs as deemed appropriate by the Manager of Day Treatment and/or Coordinator of Transitions.
• Collaborate within a multi-disciplinary team to facilitate the assessment and transition planning for individual clients as assigned for case management and monitoring.
• Engage in communication with identified clients, caregivers, other multidisciplinary team members (e.g., Day Treatment Child and Youth Workers, Teachers and Child and Family Therapist, etc.) and external services as needed to provide support and ensure effective transition planning throughout the Day Treatment process.
• Develop and implement individualized transition plans, tailored to the needs of each client, family and school while empowering clients to take ownership of their journey.
• Perform comprehensive assessments, including EMHware file reviews to inform transition planning.
• Assist and collaborate with the transitions team using an open, flexible and adaptable mindset as the program begins development.
• Travel to meet with clients, caregivers and/or other internal/external team members to ensure transition planning is consistent and implemented.
• Attend client sessions, in-team school meetings, internal assessments, admittance meetings, case reviews, treatment concerns, and other meetings as required to support transitional work.
• Submit and/or request referrals for internal services as well as external service providers to ensure continuity of care.
• Foster strong relationships with schools, community agencies and healthcare providers.
• Maintain a system of documentation and reporting that is consistent with quality assurance standards and procedures, legislation, accreditation and best practices.
• Monitor the progress of transition plans, gathers data and feedback, and assists in preparing reports and presentations for the Coordinator of Transitions and other service providers.
• Contribute to the development of data analysis to identify trends, areas for improvement and opportunities for innovation in transition planning and service delivery.
• Participate in regular supervision with the Manager of Day Treatment and engages in ongoing professional development.
• Attend and participate in required training and meetings.
• Perform other duties as assigned.
WHAT WE ARE LOOKING FOR:
• Completion of a post-secondary education in Child and Youth Care, Social Work or a related field and/or equivalent combination of education and experience.
• Minimum of three (3) years of relevant experience working in a day treatment setting, including experience in transitional planning.
• Knowledge of child and youth mental health legislation and regulations.
• Strong knowledge base of community resources to promote successful transition planning.
• Excellent interpersonal, verbal and written communication skills, including editing skills with attention to detail.
• Excellent conceptual and theoretical understanding of evidence-based practices and ability to articulate the concepts as they relate to transitions.
• Ability to liaison and build positive relationships with agency internal services/external stakeholders around ensuring appropriate transition planning is occurring in a timely manner.
• Knowledge of the developmental stages of children and families and how each stage connects to the services required.
• Competence in modelling professionalism and maintaining a high level of confidentiality.
• Excellent organizational, solution-based problem-solving and time-management skills, with the ability to manage multiple priorities and meet deadlines.
• Ability to work under pressure and in crisis situations.
• Demonstrates sound judgement related to the development of collaborative approaches working with children, youth and their families, including high-risk circumstances and safety planning.
• Demonstrate versatility in the ability to work independently and collaboratively to support agency and program goals.
• Functional technological skills (e.g., Office 360, MS Office, SharePoint, software applications).
• Awareness and appreciation for the dynamics of equity issues (diversity, equity and inclusion) especially as it relates to gender, ethnicity, race, and economic status.
• Identification with the strategic mission, vision and values of the organization including inclusion, collaboration, people centred, knowledgeable and courageous.

NICE TO HAVES:
• Applicants with fluency in another language are encouraged to apply.
• Fluency in American Sign Language.
• Therapeutic Crisis Intervention (TCI) certificate.
• Mental Health First Aid certificate.
ON-BOARDING REQUIREMENTS:
• Broad Police Record Check (within the last 180 days).
• First Aid Certification (CPR/AED Level C).
• Valid Ontario Driver’s License and access to a reliable motor vehicle.
• Completion of a Driver’s Abstract.
• Proof of automobile insurance.
